SAX: single-character read()

Upon reflection, I am quite convinced that we do not need a
single-character/byte read() method in the SAX CharacterStream and
ByteStream interfaces.

If any individual parser needs a single-character read, it can
implement a buffering layer in its SAX driver; it does not make sense
to require the application writers to implement that buffer scheme
themselves every time they create a character or byte stream.
