Actually, an XML element that is ESIS-equivalent.

I think this point is handled better by allowing Doc
in XSchema elements to go anywhere, not just at the beginning.

I think this is overkill: titles are metadata in HTML, and (Itsy Bitsy)
HTML is metadata in XSchema, so these titles would be meta-metadata.
Still, I'm not utterly opposed to it.
